This is a difficult situation. I think the best thing to do is to file the return as is and include a letter explaining what happened and asking the IRS for guidance in this situation. There isn't really any way to include only half of the FICA taxes on the return.
As other options you can enter the income from the W2 as self-employment income and pay the full amount of payroll taxes due. Then your employer would just have to reimburse you.
Or you can demand that corrected W2C. But you would need to pay back to your employer your half of the FICA in order for them to send it in and issue the corrected form.
