- Continue
- Where is this from? usually None of the Above, but select the correct answer for you
- Continue
- Select From a qualified plan
- What did you do?
- Rolled it over
- Continue
- Periodic payments?
- Select no
- Continue
- Withdrawal due to disaster?
- Select NO
- Continue
