The company you worked for changed their business identity during the year behind the scenes so you have 2 w2 forms with different EIN's and the wages should be for the part of the year you worked for each company "old" and "new". You do enter each form. 

 

The 12a concerns me. Is this a coincidence that it is the same on each w2 or did they add it all together on both? You should verify with the company what is correct before filing, unless you already know the answer.

Show the employee social security or Railroad Retirement Tax Act (RRTA) tax on all of the employee's tips that you could not collect because the employee did not have enough funds from which to deduct it. Do not include this amount in box 4.
