Sorry you're still having trouble. That nearly always fixes it for Utah filers. The usual problem with the Utah state withholding ID is the wth not being lower case. Be sure there is not another form with Utah state withholding ID, such as a 1099. If there is, it has to be lower case, too.
Sometimes recalcitrant W-2 errors can be cured by completely deleting the W-2 and then re-entering it from scratch.
