Form 8959, Additional Medicare Tax, is required when your Medicare wages reported in Box 5 of your W2, self-employment income, and/or railroad retirement income exceeds these thresholds:
- Married Filing Joint: $250,000
- Single, Head of Household or Qualifying Surviving Spouse: $200,000
- Married Filing Separate, $125,000
