There are no blank copies available because the form can't be submitted in hard copy format. It must be sent through the IRS Energy Credit Tool.
Perhaps you can show them the link to the portal so that they can take the steps necessary to register and submit the data for you to take the credit.
Unfortunately, without their cooperation, you won't be able to take the credit.
