For a 1035 exchange, the exchanging company (of the product being replaced) should issue a 1099-R with a code 6 in Box 7. These forms are entered in the section of TurboTax for retirement income (even though they may not be qualified retirement contracts) since this is the most common use of the Form 1099-R.
