Boxes 1, 3, and 5 show your wages.  Sometimes these are the same number, sometimes they are different if you have had pre-tax money withheld from your paychecks.  In your case, since all three boxes are the same that indicates that there were no pre-tax withholdings.

 

Box 2 shows the amount of Federal income tax withheld from your paychecks.  This number is based on the W-4 that you filled out when you were hired and the withholding tables that determine how much the employer should withhold.

 

Box 4 shows the amount of Social Security tax that was withheld from your paychecks.  This amount is 6.2% of the amount shown in box 3.  Based on that calculation, box 4 is correct.

 

Box 6 shows the amount of Medicare tax that was withheld from your paychecks.  This amount is 1.45% of the amount shown in box 5.  Based on that calculation, box 6 is correct.
