Yes, you'll need to include this income on your tax return. Your former employer may not have filed your W-2, but it's highly likely they did report and pay income/payroll taxes to the IRS, since you stated the employer "took taxes out". If that is in fact the case, and you do not report this income, you'll likely be getting a letter back from the IRS notifying you of this discrepancy. Better to deal with this now than later.
