Mailing / Filing my Brother's Final Return (1040)

This is quoted from IRS Form 1310

 

"Example. Assume Mr. Green died on January 4 before filing his tax return. On April 3 of the same year, you were appointed by the court as the personal representative for Mr. Green’s estate and you file Form 1040 for Mr. Green. You do not need to file Form 1310 to claim the refund on Mr. Green’s tax return. However, you must attach to his return a copy of the court certificate showing your appointment."

 

I am his executor.

 

QUESTIONS:

1) Do I leave his 1040 return unsigned?

2) On the 1040 return right now it says his name BUT my address.  Is that the correct way?  It asked for the right updated mailing address in the TT interview questions.

3) Does my name go anywhere on his 1040?  I saw a field somewhere during the TT interviews asking for MY name, but so far I can't find my name anywhere on his 1040.

4) Does his final return still get mailed to the same address it would usually go to, the regular filing address on the TT filing instructions?  The refund filing instructions on TT for his final return says for a refund send to Treasury/IRS Kansas City, MO 64999-0002.
