I suggest you contact the Broker or Agent that issued the 1099-R's
"I received two 1099 R forms for 2024. Both have the same withdrawal amount, same account number, and neither of the forms are corrected forms."
- How much did you actually withdraw from the account?
 - Did you make two withdrawals of the same amount, or only one?
 
"One of the forms have a taxable amount, federal tax withheld amount but with a different distribution number box.
The other form does not have a taxable amount, does not have a federal tax withheld amount and the distribution number box is different."
- What are the two different distribution codes.
 - Which one has the taxable amount?
 
"When I added both forms to my tax, it says that there is a duplicate form 1099 R form with the same account number. It wants me to delete one so the IRS does not send it back to me. What should I do?"

 - What do the different code mean?
