You can enter difficulty of care payments using TurboTax Deluxe.
After you enter your Form W-2 you will select "continue". The next screen you will get is "Let's Check for Uncommon Situations." You can scroll down and select the box "Nontaxable Medicaid waiver payments that qualify as difficulty of care payments".
