Not usually- you would not pay income tax on accident insurance or hospital indemnity benefits if you pay the premiums with after-tax funds. If the premiums were paid pre-tax through employer contributions (like a cafeteria plan), only the amount that exceeded the expenses would be taxable (and in most cases, if this happens you must pay back the insurance).
