Thank you for your prompt response and assistance. I did not receive the tax form yet. The insurance company's website state they are issued at the end of January 2025 on a 1099-G form. I have already received my W-2 and my former employer did not record the long term disability payments from the insurance company on my W-2. I will be receiving a separate tax form directly from the insurance carrier. Many thanks!
