There's not enough info to know what you are referring to and in what context. So I'll take a guess. Are you trying to enter a W-2 or 1099 and need to enter the Utah employer's state ID number (which should be on the form.) Each employer has their own unique Utah state ID number.
If you have Utah W-2 with income and withholding, the employer should have a Utah state employer ID that ends in wth. It should be a total of 14 characters--11 numbers and then an additional 3 letters-- most commonly wth (note: mineral withholding will have wmp instead.) Be sure to use lower case for the 3 letters. Do not use any hyphens or spaces.
Also if there was Utah withholding on a 1099, the Utah employer state ID number should be entered for the 1099--again 14 characters, consisting of 11 numbers followed by wth (or wmp for mineral). Again, use lower case.
