If this is a W-2 form and box 16 is blank or zero, leave boxes 15, 16 and 17 blank, not zero, just blank.
If box 16 has a number greater than zero in it, you can try using the EIN from box b or contact the employer for the right number.
If this is a 1099-R, and box 14 is blank or zero, leave boxes 14, 15, and 16 blank, not zero, just blank.
If box 14 has a number greater than zero in it, you can try using the EIN from box b or contact the employer for the right number.
