The amount listed for code DD in box 12 is the total cost of your medical insurance, including the amount that you paid and the amount that the employer paid. It is just for your information. It does not affect your tax return. So just enter it as it appears on your W-2. If you want an explanation of the large increase, you have to ask the payroll department.
