Why do you need a pro forma W-2? If you are doing a pro forma or trial tax return it doesn't matter what address you enter on the W-2. You're not going to file it. For the tax return that you actually file, use the address that's on the actual W-2 when you get it. You cannot file your tax return until you get the actual W-2. The actual W-2 might not match your pro forma information.
