I looked up the the Maryland's Form W-4 and it appears to be one page for 2023:  https://www.marylandtaxes.gov/statepayroll/Static_Files/Employee_W4/2023_W-4.pdf 
In section 1(c), you will check the box that applies to your status (single, Married filing separate, etc.), then if you do have dependents, you will fill out Step 3.  Maybe the form they gave you is outdated, but from what you're describing, there is a worksheet that is not available for the current W-4 for Maryland.  I would use the form in the link I posted above and turn that one in.  Thanks again for participating!
