Filing by mail--how to staple and need to include 1099?

Hi,

If one must file by mail, it looks like W-2s need to be attached.  Is this true?  Also, are all the 1099s (1099-INT, 1099-MISC, 1099-INV, etc.) also need to be included?

Also, how should the pages be stapled and organized?
