Form 4797

Instructions for Form 4797 states that the first item on line 10 should say

Trader-See attached and then we have to enter the totals for columns d, f, g in  that line.

My question is: How do I say

"trader-see attached" in line 10 of form 4797 and then the column totals?

How do I attach a separate sheet where all individual transactions are reported for the trader activity?

 

I can list all of my transactions but I am unable to write "Trader-See attached" on the first line of the table for line 10. Please help. This is my first year as a trader status. Thanks
