Yes, you need to amend your return.
Box 12 code W indicates that you have an employer-sponsored Health Savings Account and that there was money deposited into your HSA through the payroll system at work. You must always file a Form 8889 in any year you or an employer contributes money to your HSA or you make withdrawals from the account. ( TurboTax will automatically fill out Form 8889 if you and/or your employer contributed to an HSA (usually indicated by code W in box 12 of the W-2), if you used funds in your HSA (reported on Form 1099-SA), or if you inherited an HSA.)
Also box 12 code AA – Designated Roth contributions under a 401(k) plan. The contribution might qualify you for a Retirement Savings Contributions Credit( Saver's credit).
Code DD amounts- The total cost of what you and your employer paid for your employer-sponsored health coverage plan, they don't affect the numbers in your tax return.
