You generally have to include copies of forms on which you have tax withheld (like W-2, 1099-R, 1099-MISC, etc.).
You can't withhold tax on a 1099-SA, so I would say no.
P.S., See Instruction Booklet on page 6: "Completed the Income Statement Detail section where Georgia income tax was withheld on Forms W-2s, 1099s, G2-RPs, G2-LPs, G2-As, and/or G2-FLs. Attached/included the forms with my return." But, this refers ONLY to forms where Georgia tax was withheld.
