It sounds like your 8606 may be off- if you look at the form, you would have the $6000 for 2022 on Line 1 and Line 8 and if there was only $500 at the end of 2021, that is the amount that should have been carried forward. See Form 8606. You may want to see if you can get a copy of the tax returns for the last few years to see if you can find the first date the $2772 came from.
