If it is not showing in LA, the federal section was not filled out completely.

Please follow these steps:

return to the federal section,
go to income,
select the 1099-R,
go through the entry process
IRA box is blank
select not a safety officer
be sure that you select your state, mark LA
select Federal retirement, middle option, code 04E
select Qualified Plan
Select cashed it out
Select Yes, regular payments
Select start year
Answer federal box 2a question

LA will then handle the retirement correctly.
