I got a wage claim check from my old company that went bankrupt but no W2

A company I worked for in 2017 went bankrupt and didn't pay out the last few paychecks. I went through the bankruptcy court to submit a claim for the lost wages. I finally got a check for those 2017 wages last year (2022) from the bankruptcy trustee. The trustee is not issuing a W2 and the company no longer exists to issue a W2. The wage claim pay voucher had federal income taxes, FICA, and Medicare already taken out. How do I claim this on my tax return?
