Have you tried deleting the form in TurboTaxOnline?
Sign in to TurboTax and open your return by selecting Continue or Pick up where you left off
- From the left menu, select Tax Tools
- Select Tools
- In the Tool Center, choose Delete a form
- Select Delete next to KS-40SVR
- Click YES to confirm the deletion
- Resubmit your tax return
