Yes, we were covered by a high-deductible family plan for all of 2021 with no other coverage. The contributions were $700 through my employer the rest was deducted from my pay. I do not see a "line 1" on the 5329. What I filled out is Part VII and line 48 has the sum of $8,200 and subsequent to it is a "smart" worksheet with $56,084 for the current value of the HSA. Line 49 said the additional tax amount is $492.
