I just received my form at the end of August and have no clue how to read it. It says Yes to Part I, II, III, IV, IX, and X. Any help would greatly be appreciate it! I do not know where to enter this as this is the last of the information I was waiting for and why I filed an extension. Thank you in advance for your help!
