Yes, you need to report your rollover of your 403(b) to a Roth IRA on your Tax Return. You should have received Form 1099-R reporting this transaction.
If you have not received Form 1099-R yet, you should contact the Financial Institution that held your 403(b) to request a copy. You can also try accessing it online through the Financial Institution's website.
