Can I file a IRA withdraw of $100,000, a rollover of $20,000 with in 5 days back to the IRA, a Roth conversion of 30,000, and a distribution of $50,000 using one 1099 R?

Retirement tax questions
No. You need to separate it into two 1099-R's. One for the $30K in box 1 and 2a for the Roth conversion and one for $80K for the rollover and partial distribution. Rollovers and conversions cannot be on the same 1099-R.
