How do I identify the plan of retirement that I made the withdrawal from? Its on a 1099-R. Would it be a "Qualified Retirement Plan" or Non-qualified annuity?

Retirement tax questions
I'm not sure about the online version, but in the desktop version it gives 3 examples of a qualified plan, and three examples of a non-qualified plan, on that screen where you have to make that selection. Generally, the examples are sufficient for most to figure out and know what they have. But it can get tricky with annuities.
