Should i include form 1099r on my tax return if I did not take a distribution from my Roth IRA?

Yes, if you received a 1099-R form then include it on your tax return. The form is also furnished to the IRS.

It is the box 7 distributions that will determine the tax (if any) treatment and the flow to your return.
