I have 2 1099R's. I have one regular one and one that says a substitute 1099-R Do I need to select Substitute 1099-R and fill out the form 4852?

No, do not mark "I need to prepare a substitute 1099-R."
In this case "Substitute Form 1099-R" simply means a Form 1099-R not prepared on an official IRS-approved printed form. The payer has provided the same information in their own proprietary format and is to be treated the same as if it was on the official IRS Form.
