Form 8889 may be incorrect on my return.
Line 9 of the form 8889 is showing that employer contributions to the HSA are $xxx and line 2 shows $0. My wife made all of the contributions to her HSA and the employer made none.

This is actually correct.
It is an IRS rule that all contributions made by your employer and by you (or her) through a payroll deduction program are called "employer contributions". This is because they are treated the same way (both amounts are removed from Wages in boxes, 1, 3, and 5 on the W-2).
The only contributions that would appear on line 2 would be those contributions that your spouse contributed directly to the HSA outside of her employer.
