I have a HSA account but have never received a 1099-sa before for my accountant. Do I need to fill out a form for this?
I would show her my last statement and she would take info off of that

Deductions & credits
You would receive a 1099-SA form if you took distributions from your HSA funds to pay for medical expenses. If you took distributions, and did not receive a Form 1099-SA, contact the HSA custodian.
