Can I count the spouse surcharge for the health insurance premiums?

It depends on how the premiums were paid. If they were paid with pre-tax dollars, then no, you cannot count the surcharge for the health insurance. If they were paid using post ta, or after tax dollars then you can.

If they were paid after tax then where would this be recorded on taxes. My Company paid the health insurance pretax but not the spousal support

You can deduct Medical expenses if you itemize your deduction and if they exceed 7.5% of your adjusted gross income (AGI). To deduct your spouse's Health insurance premium paid with after tax dollars:
- Log in to your account.
- Go to Federal Taxes.
- Go to Deductions and Credits.
- Go to Medical and click show more.
- Go to Medical Expenses.
