May I suggest you skip the 9465 in the TT program by choosing to mail in a check ... then either send a payment of some kind and wait for the IRS bill and fill in the 9465 then   OR  do not mail in any payment and  fill out the 9465 option directly online @  https://www.irs.gov/payments/online-payment-agreement-application where the fee is cheaper.
