I had a 403b account which I closed and rolled over to an IRA when I retired in May 2019. I have a 1099-R for the IRA for my taxes. Do I need a 1099-R for the closed 403b?

Yes you do. If you do not report it as a rollover then the IRS will assume that it was not rolled over and send a bill for the tax.
You should have already received the 1099-R form the account trustee. If not then call the trustee.
