on 1099-R turbo tax form there is a place where it says Medical Insurance premiums deducted on Schedule A
can i put my medical premiums on that even if the issuer didn't fill in that field?

Hello. Yes, you can deduct medical insurance premiums that you paid for yourself even if it's not on the 1099-R. The portion that isn't deductible is any of the insurance premiums paid by an employer.

You will report the amounts not on the 1099-R, but on the "Deductions & Credits" section. There will be a section for "Medical Expenses."
on what form is the Deductions and Credits and then Medical Expenses?
The medical expenses will appear on Schedule A if your deduction is large enough to itemize. Otherwise you'll be taking the standard deduction. If you are taking the standard deduction then you won't have the Schedule A with your tax return.
