Can you please tell me why Form 8606 was not generated although my 2018 return included a 1099-R for a traditional IRA to Roth conversion?

Enter a 1099-R here:

Federal Taxes,
Wages & Income
I’ll choose what I work on (if that screen comes up),
Retirement Plans & Social Security,
IRA, 401(k), Pension Plan Withdrawals (1099-R).

OR  Use the "Tools" menu (if online version under My Account) and then "Search Topics" for "1099-R" which will take you to the same place.

Be sure to choose which spouse the 1099-R is for if this is a joint tax return.
Be sure to pick the correct 1099-R type: Standard 1099-R, CSA-1099-R, CSF-1099-R, RRB-1099-R.

If this was a rollover or conversion, answer the question that you moved the money to another retirement account (can be the same account). The screen will open up with choices of where it was moved.

[NOTE: When you get to the "Your 1099-R Entries" screen where you can add another 1099-R, use "continue" to keep going as there are additional interview questions after that screen in most cases. You can always return as shown above.]

Did you report the 1099-R for the Roth conversion in the 1099-R interview and  answer the question that you moved the money to another retirement account and then on the check that it was converted to a Roth IRA?

If you entered as below the the 8606 would have been produced.

Was this actually a rollover to a Roth IRA from a traditional account in a qualified retirement plan like a 401(k), not a conversion from traditional IRA (IRA/SEP/SIMPLE box not marked on the Form 1099-R)?  Only Roth conversions from a traditional IRA (IRA/SEP/SIMPLE box marked) are reported on Form 8606.  If it was from a traditional IRA, perhaps you failed to mark the IRA/SEP/SIMPLE box on TurboTax's 1099-R form.

Thanks for the responses!
My 1099-R was actually imported from my brokerage firm and was marked correctly. Evidently I inadvertently answered the question regarding the conversion to a Roth incorrectly so the 8606 wasn't generated. I've now corrected my mistake and sent the completed 8606 to the IRS.
