I was supposed to receive refund direct deposit but paper check received. Refund transfer processing fee of $39.99 still appears. Request refund fee as it did not occur.

Sorry the refund transfer fee is not to get direct deposit but to have the fees deducted from your refund.  To do that, your refund has to go though a third party bank.  It's true you have to set up direct deposit but that doesn't guaranty you will get it direct deposit.  Apparently your bank or routing number was wrong and the bank rejected your deposit so they had to mail you a check.  And as you can see they charged you another $20 for mailing a check.

You will not be charged another refund processing fee.  That fee comes out of your federal refund, not your state refund.

If your federal Direct Deposit bank number was wrong and the bank rejected the deposit the state deposit will reject too if you used the same info for it.
