Do I have to mention when I file my taxes that I withdraw from my 401k the prior year?

I withdraw my 401k on 2017 because I loss my job. I file my taxes and paid the fees. Do I have to mention that when I file my taxes this year again?  There's a section that ask if I received distribution after 2015 from an IRA or Roth IRA I have to put what I withdraw from my 401k?

When you do the 2018 there is nothing you have to do with the 2017 withdrawal from the 401K.

Now if you took more out during 2018, then you would get a new 1099R for 2018, and that would need to be on your tax return.
