How to treat 401(k) contributions and health insurance premiums for different wage bases?

401k contributions & health insurance premiums are included as Social Security, Medicare and local taxable wages in OH but not for Federal, OH, FUTA, or OH Unemployment Compensation taxable wage bases. Is this correct? Please post your source. Thanks.

Yes, that is correct. That's why box 18 of your W-2 equals box 5.

Actually, the larger  tax base is for  local city tax, but not local School District income tax.
