*You* do not "fill out" a 1099R for yourself. When you have a distribution from a retirement account, the financial institution that handles your retirement account issues the 1099R. Since this is a Sunday, it is unlikely that you could get your distribution accomplished today, even if you request it online. More likely it would be dated for the next business day--which will be in early 2018. If the distribution is not posted until 2018, then you will not get a 1099R until January/late February of 2019; the distribution would go on your 2018 tax return which you file in 2019.
