Form 5329 - Name of Individual

I am filling out a 5329 to ask for a Waiver of Excess Accumulation Tax because I forgot to do my minimum distribution for 2016 for an inherited IRA.  I file jointly with my husband, but the IRA account is mine.  Whose name & social security do I put on the first line of 5329? Thanks.

Form 5329 - Name of Individual

Since you are the beneficiary and the RMD should have been made to you individually (and, presumably, has now been made), the Form 5329 is to have your name and SSN only.  If you are filing it with a Form 1040X, the Form 1040X will have both your name and your husband's name, the same as on your originally filed joint tax return.
