Form 982, Discharge of Indebtedness and Insolvency Exclusion

Regarding cancellation of debt and not including as income due to insolvency exclusion, it indicates I am supposed to provide supporting documentation.  I don't know how or where to attach the insolvency worksheet to either Form 982 or the 1040 in general.
Any assistance would be appreciated.
