I have a 5498-SA form and not a 1099-SA what is the difference?

on my w-2 my employer put down HSA contrabutions but I don't have a conventinal HSA account.  I got a 5498-SA form and since I entered the anount in box 12b Turbo Tax is looking for this form,  What do I need to do?
    You only get a 1099-SA when you have distributions.  The 5498-SA is for contributions.

    Go to Federal Taxes, Wages & Income, Explore on my Own, Less Common Income, 1099-SA
    • I'm in the same situation when I get to the section about 1099-SA, what do I do? I didn't get a 1099-SA form.

      If you took no distributions in 2011, click "Skip 1099SA"  If you took distributions, get a copy of the 1099SA from the plan administrator
