The form 8995 is used to figure your qualified business income (QBI) deduction. This form can be triggered by the following:
Business income (1099 income);
An amount in box 5 of a form 1099-DIV; or
Form K1 from a Public Traded Partnership.
If you do not have any of these types of income, you can delete Form 8995 using the following steps:
Click Tax Tools in the black bar on the left side of the screen.
Click Tools.
Click on Delete a form.
Click the Delete box next to Form 8995.

If you are using the desktop version of TurboTax, make sure your program is up-to-date.
In TurboTax CD/Download, you will be prompted to download any updates available when you open the program. You can also update the program while you have it open by following these steps:
Click on the Online link in the black bar at the top of the page.
Click on Check for Updates.
TurboTax will download and install any updates available. If there aren't any updates, you will receive the message "Your software is up to date."
TurboTax Online automatically updates the website when the forms become available.

You can prepare a new W-4 in TurboTax by following these steps:
Click Other Tax Situations.
In the Other Tax Forms section click the Start box next to Form W-4 and Estimated Taxes.
Continue through the screens, entering the requested information.
Print out the W-4 form and submit it to your employer.

For the loan origination fee to be considered "points" it must be:
Figured as a percentage of the principal amount of the mortgage; and
Clearly shown on the settlement statement (such as the Settlement Statement, Form HUD-1) as points charged for the mortgage. [The points may be shown as paid from either your funds or the seller's.]
If the origination fee was not calculated as a percentage of the principal amount of the mortgage, then it is not considered "points" and not deductible.
Do keep track of all your closing costs since they will be added to the basis of the house when calculating gain if you sell in the future.
If you do include the origination fee as points paid, enter them on the screen, Tell us about the points you paid. Mark the radio button next to Points only appear on our Closing Disclosure or HUD-1 and enter the amount in the box.

Form 5498 is not filed with your tax return. It is filed by the IRA trustee or issuer. For more information on Form 5498, please see: What Is IRS Form 5498: IRA Contributions Information? If you rolled over the distribution to a traditional IRA, the distribution should not be taxable. If you rolled over the 401k distribution to a Roth IRA, the distribution will be taxable. If you took the funds out yourself and then redeposited them into the IRA, make sure you indicate this in TurboTax. To enter your 1099-R or review your 1099-R entries please follow these steps:
Click on Federal > Wages & Income.
Scroll down to the Retirement Plans and Social Security section and click on the Start/Revisit box next to IRA, 4701(k), Pension Plan Withdrawals (1099-R).
If you've already entered one (or more) 1099-Rs, you will see a screen Your1099-R Entries. Click on the Edit link next to the 1099-R you wish to review.
If you haven't already entered a 1099-R, you will see the screen, Your 1099-R. Click Yes and then Continue.
You can either import your 1099-R or click on the I'll type it in myself box.
If you select to enter the information yourself, continue through the screens, answering the interview questions.
A few screens after the 1099-R input screen, you should see a screen, What Did You Do With The Money From 401k institution? Mark the radio button next to I moved the money to another retirement account (or returned it to the same retirement account).
Below the grey line, click the radio button next to I rolled over all of this money to an IRA or other retirement account (or returned it to the same account). Click Continue.

Please follow the excellent workaround instructions posted by dmertz above. The issue he mentions affects both the online and download versions of TuroTax.
TurboTax will automatically put the QCD designation once you go through these screens:
On the screen after the 1099-R entry screen, Tell us if any of these uncommon situations apply, mark the box I transferred all or part of this money directly to charity.
You will be asked on a subsequent screen how much of the distribution was transferred directly to charity. Enter the amount in the box and click Continue.
