I received a blank W-2 form from a former employer that I did not receive any income from in 2016. Should I enter it with the zero amount or ignore it.

You would ignore it.  Do not enter the W2 into TurboTax because it will cause an error if you enter 0.00 (zeros) or if you enter a blank W2.  It's best just to ignore it!!!
